Admissions & Registrations

Our registration department welcomes patients to PMHC. They are located just inside the East entrance along Tibbals Street. All services provided by PMHC require a stop in admissions. Upon arrival, patients are asked to verify all required information and provide us with updated insurance cards and information including, Complete Address, Phone Number, Next of Kin Name/Address/Phone Number, Employer, Family Physician, Date of Birth, Social Security Number, and Insurance Information

Insurance and Pre-Certification - We suggest that prior to admission patients be aware of the type of insurance policy currently in place. Pre-certification is the responsibility of the patient. Patients are expected to bring insurance cards and a pre-certification number (if applicable) with them to the hospital. Failure to obtain this information may result in reduced insurance benefits from the insurance company in question.

Sat-Sun 7:00 am - 9:00 pm
* Prior to 5:45 am, please enter through the ER doors
on the North side of the hospital.

Advance Directives - We encourage patients to bring an Advance Directives, such as a living will or durable power of attorney for health care, if one is in place as a decision-making guide for care-givers should the patient become unable to communicate desires of medical care. Information is also available upon admission if one is not in place.
