
Health Information Management (HIM), also referred to as Medical
Records, is responsible for maintaining medical records in a
standardized and professional manner in order to protect patient
confidentiality, while allowing adequate access to providers in order to
promote quality patient care. We strictly adhere to HIPAA (Health
Insurance Portability and Accountability Act) guidelines.

HIM provides the following services including but not limited to:

Information to state agencies concerning traumas, births and deaths

Q. Where is HIM located in Phelps Memorial Health Center?
A. We are located on the west side of the building in the hallway
between the Cafeteria and the Nurses Station
Q. How can I obtain copies of my medical records?
A. You must sign a Release of Information form in order for us to
release records to you. A parent or legal guardian must sign the release
for a child or mentally incompetent patient, with required
documentation. We require at least 24 hours, and not to exceed 30 days,
to release medical information, unless it is a medical emergency. The
form can be downloaded on this website.
Q. Is there a charge for medical records?
A. There is a charge of $0.25 per page for personal medical
records. If the records
are on microfilm (before 2000), the charge is $1.25 per page. If
medically necessary, records may be sent directly to another
physician/facility at no charge to patient.
Q. Can I get my blood type?
A. We very seldom have blood types available, as blood types are
not routinely done at PMHC unless you have had surgery where a
significant blood loss is possible. For babies' blood types, if you gave
birth at PMHC, you would have been informed by the nurse or received a
card noting the blood type for you to keep with other birth information.
If not, you must follow the same requirements for Release of
Information, as stated above.
Q. Do you have immunization records?
A. We do not have immunization records. Please contact your
physician's office for these records.
Q. How can I get a certified copy of my birth certificate?
A. At dismissal, you will be given an Application for Certified
Copy of Birth Certificate to take home. Complete the form, and mail to
Vital Records, the address of which is on the form. You must also attach
a check for $12.00, which is the current rate. Fees are subject to
