Centralized Scheduling

As a convenience to our patients and physicians, PMHC has developed a new Centralized Scheduling service staffed with Licensed Practical Nurses and Certified Medical Assistants to provide an efficient service to schedule procedures, tests and appointments. We schedule for 6 different departments at PMHC, as well as outpatient specialty clinics.

Pre-Registration
Pre-Registration is completed prior to the date of your services at PMHC in order to expedite your check in the day of your test, procedure or appointment. This is completed via phone; a staff member will contact you to update insurance and demographic information. You may click on the link provided if you would prefer to fill out the pre-registration form and mail or fax (308-995-3223) it to us prior to your scheduled visit.

***Please remember to pre-authorize with your insurance company prior to your visit if required by your carrier***

Outpatient Clinics: *We schedule for all, except Pulmonary*
    - ENT- Dr. Connely-1st and 3rd Tuesday
    - Bryan LGH Heart Institute - every Thursday
    - Nebraska Heart Institute - every other Monday
    - Midlands Cardiology-Dr Kutty - 4th Thursday

- Nephrology-Dr Islam-every 2nd Tuesday
Does not see new patients here, must see him in Kearney first. Also no patients under the age of 18.

Centralized Scheduling

Pre-Admit Appointment
If you are having any type of surgery, our staff will be contacting you via phone to schedule a pre-admit appointment prior to your surgery. We will Pre-Register you at this time as well. This appointment is usually done no more than 10 days prior to your surgery. If you are from out of town, we can make arrangements to do this on the same day as your Pre-Op physical with your physician. You may fill out the
Preoperative form or Overnight form and bring with you to your pre-admission appointment to help expedite your visit. If you are unable to make a pre-admission appointment please fill out the proper form and mail or fax it prior to your surgery

    - Respiratory Therapy
    - Cardiac Rehab
    - Same Day Surgery
    - Diabetic Education
    - Dietician

*To schedule any appointments for these departments, you must have an order from your physician*
