Phelps Memorial Health Center (PMHC) is developing a new daytime Hospitalist Program to strengthen inpatient care coordination, improve communication, and support discharge planning for patients during their hospital stay.
Phelps Memorial has started recruiting Advanced Practice Providers (APPs) for the program, and open positions are now posted. The hospitalist providers will support comprehensive inpatient care during day hours, working closely with the care team and local primary care providers to help patients and families stay informed and prepared for a safe transition home.
“We’re fully supportive of this step forward, and we will work closely with the hospitalist and the full care team to ensure patients experience coordinated, collaborative care that creates the best situation for each patient and family,” said Dr. Jeff Berney, family practice provider and CEO at Family Medical Specialties.
“Our family practice providers have been instrumental in helping us launch this program, and we are grateful for their strong support,” said Tesha Broadfoot, Chief Nursing Officer at Phelps Memorial. “We will work closely with them and the full care team to ensure patients are cared for through a collaborative approach that creates the best situation for each patient and family. This supports our commitment to always providing exceptional quality and compassionate care for the families we serve.”
The program is currently in the planning phase. Additional information will be shared as providers are hired and the program approaches launch.
