Outpatient and Self-Management Program
Diabetes is one of the most severe health problems of our time. Our education program is taught by a skilled healthcare team to teach you everything you need to know about managing your diabetes so you can live a healthier life. Participants have experienced reduced hemoglobin A1c levels and are more confident in their ability to manage their diabetes.
What You Will Learn
This multi-part course is offered at various times to accommodate individual schedules. Family members and significant others are welcome to attend at no additional cost. For those with special needs, we offer personalized instruction as needed. The charge for this program is billed directly to your medical insurance and is covered by many plans. You’ll learn details in:
Understanding of diabetes and how it affects your body.
Blood glucose monitoring and how to use the results.
Healthy eating habits.
Monitoring your medications.
Importance of exercise and activity.
Early detection and prevention of complications.
Our Diabetes Education Program is in conjunction with BryanHealth which is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.
